LocationVA: ChesapeakeJob FamilyClinical, UnlicensedJob BriefThe care manager will complete a comprehensive assessment and assist with facilitating referrals to appropriate community resources and supports specific to the focus area of the military family, while maintaining the highest degree of sensitivity and comABOUT THE POSITIONWe are currently seeking a dynamic Specialty Program Care Manager to join our team at our office in Chesapeake, WA, Arlington VA or Bellevue, WA. The Specialty Program Care Manager provides highly personalized consultation, resources and support to military families through expertise in the focus areas of special needs, parenting, peer support, military relocation and transition and relationship enhancement programs. The care manager will complete a comprehensive assessment and assist with facilitating referrals to appropriate community resources and supports specific to the focus area of the military family, while maintaining the highest degree of sensitivity and compassion. Position Responsibilities: The (specialty program care manager) will conduct a comprehensive needs assessment via telephone or video to identify areas of supportive focus for participants. Demonstrate the ability to create an action plan based on needs determined from assessment. Demonstrates knowledge of goal setting and problem solving using a peer support approach. Conducts thorough research through exploration of military and community programs that are specific to the participant's circumstances. Facilitate linkage to various supports by making 3-way calls to outside agencies and military installation resources. Manages daily appointment calendar to ensure timely contact with participant for scheduled appointments. Demonstrate ability to work within established guidelines for the specialty program. Completes required trainings as determined by supervisory staff on an ongoing basis. Completes timely documentation for all sessions in government case management system. Participates in regular team meetings and individual supervision. Demonstrates an ability to work with minimal supervision and maintains continuous communication with supervisory support. Demonstrates proficiency with the simultaneous use of various internal work systems and data application systems. Demonstrate the ability to effectively communicate with team members, participants, and stakeholders in a professional manner with a strong customer focus. Must have intermediate knowledge of coaching, family systems and peer support modalities. Must have intermediate knowledge in the area of special needs to include educational, medical, and disability laws. Must have basic knowledge of moving and transitioning as it relates to military lifestyle. Performs special projects and other duties as assigned and required by supervisory staff. Position Requirements:Education: Master's degree in Social Work, Psychology, Marriage & Family Therapy, Counseling, or other closely related clinical counseling field of study is required.Licenses: Independent clinical license preferred, but not required. LCSW, LPC, LMFT, RNRelevant Work Experience: At least three (3) years of professional work experience conducting assessments and delivering care in an Employee Assistance Program (EAP), with at least half that time in a call center environment with inbound, outbound calls, telephonic case management, phone counseling, etc. Additionally, at least two (2) years of work experience with special needs and/or child and adult disability assessment and care, experience providing family counseling, and prior military experience, either as spouse or service member, is required. Knowledge of the core principles of coaching, relationship counseling and peer support modalities. Knowledge of case management standards.
