Position Summary We are seeking an experienced Skilled Home Health Registered Nurse (RN) to provide comprehensive in-home nursing care to patients in accordance with physician orders, care plans, and regulatory standards. Key Responsibilities Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge visits Complete accurate and timely clinical documentation (OASIS, skilled nursing notes) Conduct comprehensive patient assessments and develop individualized care plans Provide skilled nursing services including wound care, medication management, IV therapy, catheter care, and patient education Communicate effectively with physicians, interdisciplinary team members, patients, and families Ensure compliance with Medicare, state regulations, and agency policies Participate in on-call rotation (if applicable) Qualifications Active Registered Nurse (RN) license in the state of Virginia Minimum 1 year of home health skilled nursing experience preferred Experience with OASIS documentation required Strong clinical assessment and time-management skills Reliable transportation and valid driver’s license Current CPR certification Ability to work independently and professionally in the field What We Offer Competitive compensation (per-visit or hourly options) Flexible scheduling Supportive clinical leadership Consistent referrals Opportunity to grow with an expanding agency Low-stress, organized operations (no chaos) How to Apply Apply through Indeed or submit your resume for immediate consideration.
